PACE is a comprehensive health and social services program that coordinates primary, preventive, acute, and long-term care for individuals aged 55 and older who require a nursing facility level of care. Services are provided at PACE Centers, in the home, and in inpatient facilities. To be eligible, individuals must live in an area served by a PACE program and be able to live safely in a community setting with support.
A team of healthcare professionals—experienced in working with older adults—develops and coordinates a personalized plan of care for each participant. This team collaborates closely with the participant and their family (if appropriate) to ensure needs are met effectively. PACE covers all services provided by Medicare and Medicaid that are approved by the care team. The program may also provide additional medically necessary services not typically covered by Medicare or Medicaid, as determined by the care team. Covered services may include doctor visits, transportation, home care, and more.
Financial Eligibility
Applicants with income over the Medicaid income limit may still qualify by establishing an Income Trust. DHS caseworkers can provide more information about this process. PACE participants with spouses living in the community are eligible for the division of spousal income and resources.
Medical Eligibility
Participants must meet the nursing facility level of care as determined by the Office of Long Term Care.
